HARTFORD, Conn. (AP) _ The Connecticut Department of Public Health is launching a plan to reduce the prevalence of chronic disease in the state.

“Live Healthy Connecticut,” a coordinated chronic disease prevention and health promotion plan, will be unveiled on May 28 at the State Capitol in Hartford.

The new initiative includes five-year objectives and strategies to reduce the burden of chronic disease in Connecticut, while also promoting better health and health equity across the state. The agency calls the objectives “ambitious yet achievable.”

Various officials will be on hand for the program’s planned unveiling, including DPH Commissioner Jewel Mullen, Public Health Advocate Pat Checko and Lucinda Hogarty, director of the Connecticut Cancer Partnership.
